Following an initial enquiry, LifePath will review the referral, provide a clear quotation and confirm the referral information and written authority required before assessment work begins.

A comprehensive first assessment to understand your medical history, daily activities, physical and cognitive abilities, upper limb function, home environment, risks, support needs, and personal goals. This provides the foundation for a tailored rehabilitation plan focused on maximising independence, safety, and quality of life.

A clear written report outlining the assessment findings, identified needs, clinical recommendations and proposed rehabilitation priorities, completed within the scope and timescale agreed in the quotation.

Collaborative identification of meaningful rehabilitation goals, priorities and practical steps to support progress, participation and independence.

Functional cognitive rehabilitation

Practical, evidence-informed strategies to support attention, memory, planning, problem-solving and participation in everyday activities.

Fatigue & energy conservation

You receive a quotation. Once accepted, LifePath confirms the referral information, consent and written authority needed.

Assessment & agreed output

The assessment or rehabilitation begins, followed by the report, recommendations or progress updates agreed with you.

For professional referrals: where another organisation is paying, written funding approval or authority to proceed should be confirmed before work starts. Legal report instructions will be agreed directly with the instructing solicitor.
