How LifePath collects, uses and protects personal information.

LifePath is an independent occupational therapy practice and is registered with the Information Commissioner’s Office as a data controller. This policy explains how personal information is handled in connection with enquiries and occupational therapy services.

2. Information collected

Website enquiries

When you use the contact form, LifePath collects the details you choose to provide, such as your name, email address, telephone number, role and brief message.

Clinical and referral information

If an enquiry progresses, LifePath may collect identity and contact information, health and functional information, referral documentation, consent records, assessment notes, reports, correspondence, funding details and information needed to provide occupational therapy services.

Depending on the circumstances, LifePath may process information because it is necessary to take steps before or perform a contract, meet legal obligations, pursue legitimate business interests, provide health or social care, protect vital interests, or because valid consent has been provided.

5. Sharing information

Information is shared only where necessary and appropriate, for example with authorised referrers, commissioners, treating professionals, service providers supporting secure business systems, regulators or authorities where disclosure is required or permitted by law. LifePath does not sell personal information.

6. Website form provider

The contact form uses Formspree, Inc., a specialist form-processing service, to receive and forward website enquiries. Details submitted through the form (such as your name, email address, telephone number, role and message) are transmitted to Formspree and then delivered to LifePath’s inbox. Formspree’s own privacy policy is available at formspree.io/legal/privacy-policy. If you would prefer not to use the website form, enquiries can be sent directly to hello@lifepath.life.

7. Security and retention

LifePath uses appropriate administrative and technical safeguards and keeps records only for as long as required for clinical, professional, legal, insurance and financial purposes. Clinical records are stored separately from the public website.

8. Your rights

Under UK data protection law, you may have rights relating to access, correction, restriction, objection, portability and erasure. Some rights may be limited where LifePath must retain clinical or legal records.

9. Cookies and similar technologies

LifePath does not currently use analytics, advertising or tracking cookies. The website uses essential browser storage only after you confirm the cookie notice, so it can remember that choice and avoid showing the notice on every page. The stored preference records that essential storage is enabled, analytics and advertising are not used, the notice version and the date of your choice. The website asks again after 90 days.

You can review the information at any time using the “Cookie settings” link in the website footer. Clearing this site’s data in your browser will also remove the saved preference.
