Clinical reporting

Reports can set out functional assessment findings, identified needs, goals, clinical recommendations and proposed rehabilitation priorities. Progress updates can describe intervention, engagement, functional change and next recommendations within the agreed reporting arrangement.

Quotation and authority

Quotations reflect location and travel, documents to review, assessment or rehabilitation time, liaison and reporting requirements. Where another organisation funds the work, written funding approval or authority to proceed should be confirmed before appointments or chargeable activity are arranged.

Consent, confidentiality and communication

The client’s consent, lawful information sharing and intended recipients are clarified before work begins. The website enquiry form should not contain detailed medical information. Clinical information is requested through an appropriate route after the referral has been reviewed.
